I do not know the name of this addon only what it looks like. I have found a PVP movie on youtube of someone with the addon which when watched the addon I am talking about is around the warrior and shows his health/rage and the health/rage/mana of the enemy...

the name of the addon is ArcHUD
you can find it on Ace SVN
or you can try Metahud (i use this one), IceHud or any other hud addon that you can find on wowinterface
