In order for Omen not to be a resource hog, Omen only stores threat information on the mob you are currently targeting. The "??" will turn into actual numbers when X (default 8 seconds) seconds has passed without you switching targets....

Actually the default in TomTom is not alt-right click (configurable in TomTom), I know this since I use TomTom - they do not conflict by default.
