I've not yet seen this happen. All my chars have small shoulders ;)
Not sure if this fixes the problem, too ... I've not yet looked into this but if it's possible to un/-equip shoulders in one go this sh...

You need to restart wow for this addon.
I dont use map addons. Perhaps those of you who have problems use a mapreplacement ?
I uploaded v.4 with Morskers debug-code active and delayed the start to ADDON_LOADED.
