The editors probably just thought it was an acronym that you spell out, like A.D.D. You don't read it as "add", you read it as "a-d-d". Just goes to show that they didn't know what you were talking about.
 I wonder if they do stuff like that for scientific publications?  In any case, I'd alert the publisher in case they can fix it for future printings...
