Changed Enable at all times function to reduce lag when selected. Note this is now On by default.
Bug fixed.
Aggro indicator is something I considered before, it's not an easy thing to do.
At the moment it's just getting the current version stable.

In recent versions I have been pushing to improve performance of the mod and have made huge improvements but this has introduced new bugs which need to be fixed.

When players are out of sight (>100 yards), they are put on a reserve list. While on the list Healbot will not do any range checking on them and ignores them. Players are taken off the list in the same order they are put on.

Yes, uncheck SmartCast or from 2.21 you can check an option to cast "enabled" setting if the target is in combat.

It is possible you have set spells for the bars in an enabled state and are clicking on them while disabled.
Go back to the spells tab and check the Set Spells For dropdown.
