using AloftCastWarning pre-cata had this same problem.

To fix this, open "Aloft/Aloft/AloftCastBar.lua"
Go to line 513.

Same for me. The name sometimes doesn't show on freshly created nameplates.
When you turn the camera so the nameplate goes out of screen an back on again, it works fine.
