Updated for 24-12-14: Added visual for Prismatic Crystal. Changed Hearthstone to Garrison Hearthstone in the Portal Menu.

Portal menu
The portals in the download are only for alliance cities. Import this string in WeakAuras for horde cities:

The pictures of the cities won't be changed, but if you click them they will take you to these respective cities:
Stormwind > Orgrimmar
Ironforge > Undercity
Darnassus > Thunder Bluff
Exodar > Silvermoon
Theramore > Stonard
Stormshield > Warspear

To open the menu make sure you have the correct profile set in kgPanels. Then paste this into a macro and use it to toggle the portal menu:
